The Sustainability and Transformation Plan (STP) is a comprehensive five-year plan to transform health and care services for local people so they are fit for the future.
These plans (44 in total) are being produced all across England to address how local services will evolve and become sustainable over the next five years and beyond. These are the plans that will deliver NHS England’s Five Year Forward View – a vision of better health, better care and improved NHS efficiency.
As such, Suffolk and North East Essex STP plan is the local implementation of a national strategy and it supports the core NHS priorities which are:
The Suffolk and North East Essex STP plan was submitted in 2016 and describes how health and social care organisations will work together differently, to deliver the necessary change over the next five years. A key theme within the plan is an increased focus on preventing ill health and promoting peoples’ independence through the provision of more joined-up services in or closer to peoples’ homes.
